RESUMEN Introducción: La enfermedad renal crónica (ERC) representa un problema de salud global con alta morbimortalidad, siendo fundamental su detección temprana para prevenir su progresión. La estimación de la Tasa de Filtración Glomerular (eTFG) ha sido el marcador principal para evaluar la función renal; sin embargo, la albuminuria, medida a través del Cociente Albúmina/Creatinina urinario (CACu), ha demostrado ser un predictor más sensible de daño renal temprano. Objetivo: Evaluar la relación entre el CACu y la eTFG como predictores de riesgo renal mediante la escala KDIGO 2024 en pacientes atendidos en el Servicio de Medicina Interna del Servicio Autónomo Docente Hospital Central de Maracay (SADHCM). Metodología: Estudio cuantitativo, descriptivo, de campo y transversal con 73 pacientes (64 % femeninos, edad promedio 55,62 años). Se determinaron niveles séricos y urinarios de creatinina, albuminuria, y se calculó el CACu y la eTFG (CKD-EPI 2021), clasificando el riesgo renal según la escala KDIGO 2024. Resultados: El 43,80 % de los pacientes presentó albuminuria moderadamente aumentada (A2), mientras que solo el 5,50 % mostró eTFG disminuida (G3a). El 45,20 % de la población se clasificó con riesgo moderadamente elevado y el 4,10 % con riesgo alto según KDIGO 2024. Las comorbilidades más frecuentes fueron sobrepeso (67,12 %), antecedente de COVID-19 (54,79 %) e HTA (39,73 %). Conclusiones: El uso combinado del CACu y la eTFG mediante la escala KDIGO 2024 permite identificar de manera temprana a pacientes con riesgo renal, a pesar de tener un eTFG normal o alta.
ABSTRACT
Introduction: Chronic kidney disease (CKD) represents a global health problem with high morbidity and mortality, making early detection essential to prevent its progression. The estimated glomerular filtration rate (eGFR) has been the primary marker for assessing renal function; however, albuminuria, measured by the urinary albumin/creatinine ratio (UACR), has proven to be a more sensitive predictor of early kidney damage. Objective: To evaluate the relationship between UACR and eGFR as predictors of renal risk using the KDIGO 2024 scale in patients treated at the Internal Medicine Service of the Autonomous Teaching Hospital of Maracay (SADHCM). Methodology: A quantitative, descriptive, field, and cross-sectional study was conducted with 73 patients (64 % female, mean age 55.62 years). Serum and urinary creatinine levels and albuminuria were determined, and the creatinine clearance (CBC) and eGFR (eGFR) were calculated (CKD-EPI 2021), classifying renal risk according to the KDIGO 2024 scale. Results: 43.80 % of patients presented with moderately increased albuminuria (A2), while only 5.50 % showed decreased eGFR (G3a). 45.20 % of the population was classified as having a moderately high risk and 4.10% as having a high risk according to KDIGO 2024. The most frequent comorbidities were overweight (67.12 %), history of COVID-19 (54.79 %), and hypertension (39.73 %). Conclusions: The combined use of CBC and eGFR using the KDIGO 2024 scale allows for the early identification of patients at renal risk, even with a normal or high eGFR.
