RESUMEN La reparación tisular en pacientes con patología vascular representa uno de los desafíos más complejos de la medicina regenerativa. En condiciones de diabetes mellitus y enfermedad arterial periférica, así como en la insuficiencia venosa crónica, el microambiente de la herida se ve comprometido por una hipoxia crítica y una desregulación metabólica que detiene el proceso de cicatrización en una fase inflamatoria persistente. La oxigenoterapia hiperbárica (OHB) ha emergido no sólo como un suplemento de oxígeno, sino como un potente modulador genético y enzimático capaz de revertir este estancamiento. Para ello, se realizó una revisión narrativa mediante una búsqueda avanzada en PubMed/MEDLINE, ScienceDirect y Google Académico, seleccionando 24 artículos de alto impacto (2019-2026). Se priorizaron metaanálisis, revisiones sistemáticas y ensayos clínicos, con el fin de describir como la hiperoxia intermitente estabiliza el factor inducible por hipoxia 1-alfa (HIF-1α), activa vías de señalización de reducción-oxidación (redox) y modula la respuesta inmunológica para facilitar la transición hacia la fase proliferativa y de remodelación. La evidencia actual sugiere que, si bien la OHB es un adyuvante decisivo en la reducción de tasas de amputación en el pie diabético mediante la estabilización del HIF-1α y la modulación redox, su éxito en la patología venosa está estrechamente ligado a la resolución previa de la hipertensión venosa de base.

ABSTRACT Tissue repair in patients with vascular pathology represents one of the most complex challenges in regenerative medicine. In conditions such as diabetes mellitus and peripheral arterial disease, as well as chronic venous insufficiency, the wound microenvironment is compromised by critical hypoxia and metabolic dysregulation that halts the healing process in a persistent inflammatory phase. Hyperbaric oxygen therapy (HBOT) has emerged not only as an oxygen supplement but as a potent genetic and enzymatic modulator capable of reversing this stagnation. To this end, a narrative review was conducted through an advanced search in PubMed/MEDLINE, ScienceDirect, and Google Scholar, selecting 27 high-impact articles (2019-2026). Priority was given to meta-analyses, systematic reviews, and clinical trials to describe how intermittent hyperoxia stabilizes hypoxia-inducible factor 1-alpha (HIF-1α), activates reduction-oxidation (redox) signaling pathways, and modulates the immune response to facilitate the transition toward the proliferative and remodeling phases. Current evidence suggests that, while HBOT is a crucial adjunct in reducing amputation rates in diabetic foot by stabilizing HIF-1α and modulating redox, its success in venous pathology is closely linked to the prior resolution of underlying venous hypertension.
