Hallazgo mediante microscopía especular en la distrofia endotelial de Fuchs: a propósito de un caso

Resumen


RESUMEN La distrofia corneal endotelial de Fuchs (DEF) es una patología degenerativa bilateral y progresiva que constituye la distrofia corneal posterior más prevalente y una de las principales indicaciones para trasplante corneal en adultos. Se presenta típicamente en la quinta-sexta década de vida con predilección por mujeres (3:1 a 4:1), caracterizándose por pérdida progresiva de células endoteliales, formación de guttas y edema corneal de predominio matinal. El diagnóstico requiere evaluación integral combinando biomicroscopia con microscopía especular como técnica de referencia, permitiendo cuantificar la densidad celular endotelial y evaluar polimegatismo y pleomorfismo. Objetivo: Precisar los hallazgos clínicos y morfométricos característicos de la DEF en una paciente de 64 años, analizando la correlación entre densidad endotelial y pérdida de regularidad óptica. Caso clínico: Paciente femenina de 64 años con disminución progresiva de agudeza visual bilateral de predominio matinal. El análisis especular reveló densidad celular crítica de 745 células/mm² en ojo derecho y 1004 células/mm² en ojo izquierdo, con ausencia total de hexagonalidad (0%) y polimegatismo severo (CV 25-55%) en ambos ojos, correlacionándose con guttas confluentes y edema corneal. Resultados: Se documentó discrepancia entre densidad celular y función visual, sugiriendo que múltiples parámetros morfométricos son predictores de severidad funcional. Conclusión: La microscopía especular es herramienta diagnóstica indispensable para detección precoz de cambios endoteliales subclínicos, permitiendo estratificación de riesgo quirúrgico y planificación terapéutica personalizada en pacientes con DEF avanzada, enfatizando la importancia del diagnóstico temprano y vigilancia oftalmológica rigurosa para preservar función visual y mejorar calidad de vida.

ABSTRACT Fuchs endothelial corneal dystrophy (FECD) is a bilateral progressive degenerative pathology that constitutes the most prevalent posterior corneal dystrophy and one of the main indications for corneal transplantation in adults. It typically presents in the fifth-sixth decade of life with female predominance (3:1 to 4:1 ratio), characterized by progressive loss of endothelial cells, guttae formation, and corneal edema with morning predominance. Diagnosis requires comprehensive evaluation combining slit-lamp biomicroscopy with specular microscopy as the gold standard technique, allowing quantification of endothelial cell density and assessment of polymegathism and pleomorphism. Objective: To characterize the clinical and morphometric findings of FECD in a 64-year-old female patient, analyzing the correlation between endothelial density and loss of optical regularity. Clinical case: A 64-year-old female patient with progressive bilateral visual acuity reduction with morning predominance. Specular analysis revealed critical endothelial cell density of 745 cells/mm² in the right eye and 1004 cells/mm² in the left eye, with complete absence of hexagonality (0%) and severe polymegathism (CV 25-55%) in both eyes, correlating with confluent guttae and corneal edema. Results: A discrepancy was documented between cell density and visual function, suggesting that multiple morphometric parameters are predictors of functional severity. Conclusion: Specular microscopy is an indispensable diagnostic tool for early detection of subclinical endothelial changes, allowing surgical risk stratification and personalized therapeutic planning in patients with advanced FECD, emphasizing the importance of early diagnosis and rigorous ophthalmologic surveillance to preserve visual function and improve quality of life.
